USAF: Pilot Errors, Unhealthy Safety Culture Caused B-1B Loss

The U.S. Air Force on July 25 released an Accident Investigation Board report into the Jan. 4, 2024, crash of the Rockwell Collins bomber at Ellsworth AFB, South Dakota, that destroyed the aircraft at a loss of $456.2 million.
